You enter the emergency room due to an illness, receive the necessary care, possibly carry out tests and checks and, upon discharge, a receipt may also arrive together with the report. Not the traditional one, which certifies a payment, but a document intended to show how much the assistance received cost the National Health Service, even when the patient did not have to pay a single euro. It is one of the innovations contained in the budget maneuver 2027which introduces a transparency mechanism intended to change the way in which Italians perceive the economic value of public healthcare services.
The question, inevitably, is whether this new receipt will also lead to new expenses for citizens. The answer is no: the measure does not introduce a new ticket and does not transform free services into paid services. The objective is to make visible an expense that already exists, but which normally remains unknown to those receiving treatment, because it is supported by the healthcare system through public resources.
How the health receipt foreseen by the 2027 budget works
The novelty is contained in article 69 of the 2027 budget bill, dedicated to the refinancing of the national health service. Paragraph 8 provides for the issue to patients of a summary document indicating the type of healthcare service received and the related tariff, through a system that should make the costs of assistance more immediately understandable.
The document may be paper or digital and will concern the services provided by public and private healthcare facilities accredited with the National Health Service. It will therefore not be a simple administrative receipt, but an information tool through which the patient will be able to know the tariff value of the care received, regardless of any financial contribution requested.
To understand its significance, just think of a specialist visit or a diagnostic test carried out in a public facility. Today the citizen generally knows the amount of the co-payment, when applicable, but hardly knows what the tariff recognized by the healthcare system is for that same service. With the new mechanism, this information should become accessible directly at the time of service provision.
Emergency room receipt: who will have to pay and what changes for patients
The most important point concerns the distinction between cost of the service and amount actually owed by the patient. They are two figures that must not be confused, because the health receipt was created to make the first visible, without automatically modifying the second.
Anyone who receives a free service will continue not to have to pay simply because they have received the summary document. Those who are required to pay a ticket will continue to do so according to the rules already in force, while people who benefit from the exemptions provided for by the legislation will maintain the relevant rights.
As regards the emergency room, the current regulations provide for a ticket for classified accesses upon discharge with code white, i.e. code 5when they are not followed by hospitalization, without prejudice to applicable exemptions. The amounts and some operating methods may vary at regional level. The services classified with different codes instead follow the free rules established by the healthcare system.
The new receipt, therefore, does not introduce an additional payment for those who show up at the hospital, but will make it possible to know the tariff associated with the assistance received, even in cases where the cost does not fall directly on the citizen.
Which visits and tests will be affected by the new receipt
Although the focus is mainly on the emergency room, the news has a broader scope and also concerns specialist outpatient servicesincluding medical visits and diagnostic tests carried out within the National Health Service.
According to the measure, the mechanism will also affect non-urgent visits to the emergency room which do not end in hospitalization. In these cases, the patient will be able to receive a document containing information relating to the services performed and the corresponding fees.
This is information that, at least in part, already exists. The healthcare system in fact has nomenclatures and tariffs through which the amounts recognized for the various services are defined, but these tools are generally consulted by operators and professionals, while they are unfamiliar to the majority of citizens.
However, it should be noted that the reported rate does not necessarily coincide with the actual cost incurred by the individual healthcare facility. This is a value established under the pay-for-performance system, which is not equivalent to a detailed accounting of all the expenses incurred to treat that particular patient.
Because the government wants to show how much free treatment costs
The measure is based on the idea of making the economic functioning of public healthcare more transparent. In Italy, access to many services occurs free of charge or through the payment of a co-payment, but this does not mean that the treatments are free of costs: medical and nursing staff, diagnostic equipment, healthcare materials, maintenance and organization of the structures require resources that are financed mainly through general taxation.
The technical report accompanying the bill identifies the purpose of the intervention precisely in the transparency of the costs incurred by the National Health Service. Showing the tariff associated with a service therefore means making the community’s contribution to the functioning of the healthcare system more evident.
The intention is also to encourage greater awareness in the use of public services, especially in a context characterized by limited resources, organizational difficulties and growing pressure on emergency rooms.
However, a fundamental distinction remains: promoting appropriate use of health facilities cannot mean discouraging those who really need assistance. The assessment of urgency remains a clinical question and the citizen must be able to continue to contact the emergency services when their conditions require it, without interpreting the receipt as a financial penalty.
When will the health receipt arrive and what is missing to make it operational
The introduction of the health receipt is foreseen by the 2027 budget bill, but the measure is not yet definitively approved and will have to go through the parliamentary process before becoming law.
Even after possible approval, further steps will be necessary to concretely define its functioning. In fact, the text refers to a decree of the Minister of Health to establish the implementation methods of the new document, including the operational characteristics and procedures through which it will be delivered to patients.
At the moment, therefore, it is not possible to indicate a certain date from which all citizens will receive the receipt after a visit or visit to the emergency room.
The innovation could however represent a significant change in the relationship between citizens and public health, making the economic value of the services guaranteed by the national system more understandable. A way to remember that a free service for those who receive it is not a cost-free service, but the result of a collective financing system that allows healthcare to be guaranteed.
And that, at least in the intentions of the maneuver, this greater awareness must not translate into a new bill to pay.
